One Page Profile Workshop

This interactive workshop introduces participants to the purpose, structure, and practical use of One Page Profiles as a core tool in person centered practices. Through guided instruction and hands on activities, participants will learn how to gather meaningful information and create One Page Profiles that genuinely reflect what matters to a person and how best to support them. In addition to creating their own profile, participants will learn how to review and enhance existing profiles.

By the end of the session, participants will understand how One Page Profiles support Oregon’s ISP process, promote empathy and partnership, and serve as essential tools for providers, families, and teams working toward truly person centered supports.

  • Explain what a One Page Profile is, including its origins, core components, and role within Oregon’s ISP process.
  • Identify multiple purposes and uses of One Page Profiles in planning, communication, and support.
  • Describe key person centered concepts—Important TO, Important FOR, and the balance between them—and how they guide profile development.
  • Recognize the characteristics of effective One Page Profiles, including clarity, relevance, strengths based language, and person centered details.
  • Locate and utilize available resources to support ongoing development of high quality One Page Profiles.
