Person Centered Planning with Lifecourse

This interactive training introduces participants to the core elements of person centered planning using LifeCourse™ principles and tools. Through guided discussion and hands on activities, participants will explore how to gather and use person centered information, understand planning within the context of the whole family, and apply LifeCourse™ Trajectory and STAR tools to support meaningful problem solving and goal planning.
