Person Centered Planning with Lifecourse

This interactive training introduces participants to the core elements of person centered planning using LifeCourse™ principles and tools. Through guided discussion and hands on activities, participants will explore how to gather and use person centered information, understand planning within the context of the whole family, and apply LifeCourse™ Trajectory and STAR tools to support meaningful problem solving and goal planning.
Designed for individuals, families, direct support professionals, educators, and case management staff, this session provides practical strategies for helping people identify what they want for their life—and how to take the next steps toward a good life vision.
By the end of the course, participants will be able to:

  • Explain the purpose of person centered information and why it is foundational to effective planning.
  • Recognize when and how LifeCourse™ tools can enhance planning conversations.
  • Apply a whole family perspective to identify supports and opportunities.
  • Create a LifeCourse™ Trajectory to map what a person wants—and wants to avoid—in their future.
  • Use the STAR tool to explore supports in four domains (skills, technology, relationships, and strategies) to generate practical solutions.
